I figured it out! You're going to need your social security#, birthday, address, and the account#. You do NOT need the card number.
Call their customer service number, the one ending in 4169. Press 2 to continue using the automated service THEN PRESS 2 WHEN IT ASKS ABOUT RECEIVING A CARD YOU DID NOT REQUEST. You want to talk to the fraud department. This is the ONLY way to find an actual human. It says something about they don't have your account info, but they do. I sat on hold for about 22 minutes and got through to someone. I explained I never got the card, I had to give them that info I listed up top then he said they'd send a card. ASK FOR FEDEX SHIPPING. It will take $15 out of your account but it comes much faster, 3 days instead of 7, AND you get a tracking number. I asked about getting a temporary number just to transfer the money out, but he said he couldn't do that. Hope this helps.
