Submitting an amended return (CA state specific) - first page on the state (8453-OL) says DO NOT MAIL THIS FORM TO FTB should be omitted when I mail in my amended return?

I am submitting an amended return, state and federal. The instructions printed out for both, I was able to pay federal electronically, but not state. The first page on the state (8453-OL) says "DO NOT MAIL THIS FORM TO FTB" should this be omitted when I mail in my amended return. Additionally, there is not a 3582X form included. But there is a 1040 X. Help!
