There are no known issues with what you're describing.
Perhaps you could delete and re-input your information to see if this will correct the issue.
In TurboTax Online,
- Click the Federal tab in the left pane.
- Click the Deductions & Credits tab near the top of the screen.
- Under Your tax breaks, you should see Child and Dependent Care Credit. Click Edit/Add to its right.
- Proceed through the interview questions until you get to the Here are your care providers. Next, we'll get your expense info page.
- On that page, you'll see your care provider's name. To the right, you'll see a trash can icon -- click this to delete your information for this care provider.
- The same page will regenerate, and you can click Add a care provider to start over.
