After an exhaustive phonecall and searching online, the correct app is called TURBO CARD, and is issues by GREEN DOT. From there, the process is fairly self-explanatory but I'll explain below:
Using the Turbo Card app:
Beneath "Transactions" and "ATM & Reload Map", select "Settings."
Choose "Manage Card".
