No you need birth certificate(or pertenity test, or note from the person saying that they allowed you to carry the child IF you qualify to carry the child), school records(proving the child has lived with you at the same address have on the tax return for more than 6 months), lease with your name on it, Utility bill with your name on it. This need to be had for each child that you have carried.
