If it's a Turbo Prepaid VISA Card,  you can phone this customer service number.   (888) 285-4169.

 

Listen to all the menu options in the recording before entering anything.    One of the options is "To report a card lost, stolen, or never received."     Then on the next menu, you'd choose "lost."  I presume that blocks the original card and starts a process to send you another.  You'll just have to see what it says.

 

If you need to speak to someone, such as if your address has changed, then phone it again, and enter the card number at the first when prompted.
