After you file

It depends on where  you got the cash tips. If this was from your regular W-2 job, follow the instructions at <a rel="nofollow" target="_blank" href="https://ttlc.intuit.com/questions/4047959-i-need-to-file-a-4137-form-my-boss-did-not-report-my-tips"...>
The page shown in the screen shot comes up after entering the W-2.

If the tips were from a job, where you did not get a W-2, you do not use form 4137. You enter the tip income as self employment income (schedule C or C-EZ) and TurboTax generates schedule SE to pay your FICA tax (and the "employer's"  share too)
