Do I submit a Form 1040V with my check and 1040X? Also, Instructions say mail 1040-X to Austin, TX; 1040-V says mail 1040-V and check to Charlotte, NC. What's correct?

After you file
If you are sending your check with the 1040X it is not necessary to send a 1040V as well- the 1040V just ensures that the payment is applied to the correct tax account. Also, send to the Austin TX address, the 1040V address is for payments only (although if you send to the wrong address it will still get processed but there is a delay).

After you file
Do not send the 1040V. Just include a check with the 1040X. Ignore the 1040V. That is for the original return or what the original return would have been if it didn’t need correcting. Don’t pay the amount on the 1040V. Pay the amount on the actual 1040X.
