You don't enter a Form 1095-B in your tax return. It doesn't affect your taxes. Just keep it with your tax records in case there is any question about what kind of health insurance you had.
On your federal tax return you do not have to show that you had health insurance. You only have to enter health insurance information if you had insurance from the Marketplace ("Obamacare").
