"how can you show the amounts that have been revised"

That goes in the lines that you are showing as EMPTY currently.

 

Columns A Through C
Column A. Enter the amounts from your original return.
However, if you previously amended that return or it was
changed by the IRS, enter the adjusted amounts.
Column B. Enter the net increase or decrease for each line
that you are changing.

Column C, enter the correct amount based on your revised tax return.

Explain each change in Part III. If you need more space,
attach a statement.

Attach any schedule or form relating to the change.

This does not include your old 1040, nor your new 1040.

The lady was correct.
Do not include your old 1040 nor your revised 1040 because the Form 1040-X reflects any changes there and becomes your new tax return.

The only time you can leave lines 1-23 EMPTY is if no dollar amounts are changing and you are filing 1040-X for some other special purpose.
