I’m having similar problem. When asked if I received a 1099R form for 2021 I answer no. Next screen asks if you took a disaster distribution in prior year. I answered yes. Next screen asks for info from 2020 8915-E. I opted to spread tax over three year, I fill in all the info then it says I have to revisit the 1099-R retirement topic. However, I didn’t get one for 2021
