I don't know why line B4 of TurboTax's 1099-R form would have the amount from box 4.
What is the code in box 7 of this Form 1099-R?
Is the IRA/SEP/SIMPLE box marked?
You might try deleting and reentering this Form 1099-R if you have not already done so.
