@JulieS Yes, it actually is not the 1099R so to speak from my financial institution. What happens is this: I get a Check Your Entry To Make Sure Your Number Is Correct notification when undergoing the Federal Review and this is where I am taken (below)
I can't edit any numbers on this form, only the amount that was carry overed which was in fact in this fake example 40,000 of the 50,000 as 10,000 was paid in federal tax.
