The Social Security Disability payments are reported on Form SSA-1099. Form SSA-1099 is easily obtained online. An account needs to be created and the tax form can then be requested. Here is a link to the Social Security website to get the Form 1099-SSA. Medicaid is reported on Form 1095-B. Form 1095-B is not entered into the tax return and is not needed.
Some Social Security Disability beneficiaries have to pay federal income taxes on their Social Security Disability benefits, while others do not. Generally speaking, if Social Security Disability Insurance (SSDI) or Supplemental Security Income (SSI) is your only source of income, you will not have to pay any federal income taxes on your Social Security Disability benefits.
