Box 3 Proceeds from Collectibles

Box 7 Loss is not allowed based on amount in 1d
Box 14 State name
Box 15 State identification no.
Box 16. State tax withheld
Out of these, I only see value for 1a, 1b, 1c, 1d, 1e, 6
