First, did you have a Self-only HDHP policy from January through October? If so, when you indicate that you had different types of HDHP coverage for different months of the year, on the list of months on the next screen, check off Self-only for Jan through Oct, and Family for Nov-Dec. (I assume that you switched your HDHP policy to Family by November 1st).
NOTE: what is important is the type of HDHP policy you had, not whether or no your wife was on it (yet). It's common for taxpayers with a dependent to have a Family HDHP policy and the spouse to have a different policy through the employer.
If, however, you had a Family policy the whole year, then enter that you had a Family policy for the entire year.
Does your wife have an HSA (HSAs are owned by the individual, not the couple, so you each could have one)? If not, she will not be asked about her HDHP coverage, so no, she doesn't show Family coverage.
Only if your wife has her own HSA should she check the HSA box on the "Tell us about your Health-relate accounts" screen. In this case, she would indicate that she has Family coverage for November and December.
NOTE: she may see a question "What type of High Deductible Health Plan did [name] have on December 1, 2020?" (yes, 2020). In her case, she should answer NONE so that she doesn't get a coverage lapsed message.
