You don't need Form 8938 to report a withdrawal from your Canadian RRSP. Any dividends you earned are also not taxable as long as they aren't withdrawn.

To report a Distribution from a Canadian RRSP, in the Wages & Income section, scroll down to Retirement Plans and Social Security.
