On form W-2G, the state info is in 3 boxes:
- Box 13: State/Payer's state ID number
- Box 14: State winnings
- Box 15: State income tax withheld
If box 15 is 0 or blank, you do not have any state tax withheld and you should leave all 3 boxes 13, 14 and 15 blank, even if box 13 and/or 14 is populated.
