Are you referring to County Codes for Indiana’s Schedule CT-40, County Tax Schedule for Indiana Residents?
If so, they’re displayed on page 2 of that form, which can be found at 2021 Individual Income Tax Forms - DOR. Since Indianapolis is in Marion County, the relevant County Code would be 49.
