Yes, you can claim your Medicare Part B payments on your tax return.   No, you do not claim them under your spouse's Form SSA-1099 Box 5.  

 

To claim Medicare Part B premiums when you are not receiving social security directly under the Medical Expense screen for Itemized Deductions.  To enter the Medicare Part B premium along with any other qualified medical expenses

 

  1. Start by clicking on the Federal Taxes tab on the left side of your screen. 
  2. Selection Deductions & Credits at the top of the screen.
  3. Scroll down to Medical and click on Medical Expenses.
  4. Follow the prompts to enter your premiums for your Medicare Part B.

You can enter medical expenses for both you and your spouse.   Be careful not to duplicate.   Do not enter the premium for your spouse that is shown on his SSA-1099.   Only enter additional expenses your spouse may have.
