Which of these types? (your form should say)
1099-R (Retirement plans like 401k's, IRAs, pensions, and annuities)
CSA 1099-R (US Govt Civil Service retiree annuity)
CSF 1099-R (US Govt Civil Service survivor annuity)
RRB 1099-R (railroad annuities and pensions)
