Do you have such a number on any other form? What kind of 1099 is it? You said "unemployment," so is it a 1099-G? Does your number end in wth (most commonly)? Some end in wmp but that's for mineral production withholding.
Several users have reported they had to use lower case for the wth for the Utah Payer withholding number. It should be entered without dashes. You need to do this on all forms that have a Utah withholding number that ends in wth: W-2, certain1099's, etc. Do you have it on any other form?
Since you've played around with it a number of times, if it were me, I'd delete the 1099 completely and re-enter the 1099 info from scratch, using lower case for the last 3 characters. Sometimes deleting a form completely will reset something.
