Hello, so I took the full 100k from my 401K using the CRD act. I'm filling out my taxes online and I got to the part where I'm reporting this information. I answered all the questions and confirmed I qualified. I got to the part where it says do u want to spread the payment within 3 years or pay it all now. The only thing on that page was a check box saying if u want to pay it all now. But I didn't so I left that box blank. But the form I need 8915-E in order to spread the payment within 3 years is still not coming up. How do I get that form?
