Sorry, two part response to my original question so I hope I don't lose you...

 

I jumped on the state's tax website and started to fill out the state return online.  Last year I had a refund of $317 from state.  I did not receive a 1099-G.  I only see 2015 and 2016 - 1099-G links but nothing for last year.  I saved last years statement from the check voucher.  I received that refund on 4/20/2020.  I didn't receive a 1099-G via USPS either.  Do I not have to enter this as income on the website?

 

Back to my original question, Ohio Department of Job & Family Services sent me a 1099-G for unemployment benefit of $5,496.00 and they reported the amount for the Federal withholding.  That included their Fed Tax id number.  They did not withhold state tax so those boxes were left blank.  (withholding and OH State tax Id no)  When trying to include this on the State filing form online, it would not allow me to add this 1099-G without the OH State Id no.   This was my original question.  Hope I didn't confuse you.

 

I could let TurboTax file my state, but I thought I would save the $50 and file it myself.  Can you tell I have never done this before??
