Thank you for that encouragement, and for reporting this issue for further investigation.
I would suggest a piece of evidence that I observed. The names which appeared in the wrong places were, in all cases, names on forms which had previously been deleted. These were forms imported from the previous year but were not used in the current report, so I deleted them. I hope that might be a helpful clue.
