I downloaded forms from investment firm and filled in fine. At review, a box popped up to say enter "ordinary, capital gains, non-taxable dividends or exempt interest dividends for T Rowe Balanced Fund". Refund was significantly reduced. It seems like the same numbers have been entered twice.
