@ESO, I took @WendyN2's advice:
- faxed Form 14039
- in TurboTax, I "un-indicated" I was a victim of identity theft, so the affidavit was removed
- e-filed through TurboTax
The e-file seemed to transmit properly as I received an email from Intuit saying the IRS had accepted my return.
