I have contacted Ambetter superior regarding 1095 A. Here is what they said:

I will be happy to provide you the information needed. You can obtain your 1095-A form from your Healthcare.gov web account. If you do not have one please contact the Health Insurance Marketplace directly at [phone number removed]. Unfortunately Ambetter does not have or send the 1095-A forms.
