What you do is ask whoever issued the 1099-MISC to give you a corrected Form 1099-MISC that is filled out properly. Did they actually withhold state tax from your payment? If so, box 17 has to be filled out. Box 18 is not required. If they did not withhold state tax, box 16 should be empty.
