If box 15 is blank on your Form W-2, that is fine. Just leave it blank in TurboTax. You should enter your Form W-2 as it appears. Lack of any information in boxes 15-17 probably means you are in a state that does not have income tax.
Make sure that you do not select a state from the dropdown box either.
