For the IRS' instructions on how to handle the situation where the employer will not send you a W-2, please see this IRS webpage.
An alternative is to see if you can get a transcript of the income forms sent to the IRS (your employer should have sent copies of your W-2(s) to the IRS).

Note that transcripts are not available by mail at the moment, but are available only online.
