They did this in an unusual way but it is OK. Just enter the 3rd correct 1099-R with the box 1 and 2a the same.
What they did was to issue a corrected 1099-R that cancelled the first as if the first never happened. Then they issue a NEW original 1099-R to replace the first that was now cancelled - so now you only have one original 1099-R to file
