The description is Uber 1099-K and the amount/income is the total of gross receipts received. It goes under General/Additional/Other Self-Employment income not reported on a 1099-MISC. You do not enter each box of a 1099-K, only the total gross receipts or sales.
- Click Business or Self-Employment, then Continue

- Business Income and Expenses, click Start/Update
- Let's confirm your business info, Continue
- Enter profile info if not done already
- Your Business page
- Go to Business Income - Start
- General Income (Additional or Other SE Income), click Start
- Type in your description, click Continue
