You can delete the Lump Sum Credit Worksheet form from the Tax Tools menu:
- Click on Tax Tools near the bottom of the left side menu.
- Select Tools.
- Click on Delete a form.
- Find Lump Sum Retirement Credit Wks under the Ohio Return on the list, click Delete to the right.
- Confirm that you want to delete the form.
- Click Continue.
