You don't. Form 1095-C merely shows that you were offered and/or enrolled in self-insured health coverage for 2016. Unless you also have a Form 1095-A (i.e., you were enrolled in an ACA exchange plan), you do not need to enter anything from Form 1095-C (or Form 1095-B, for coverage other than self-insured & exchange plans) other than the fact that you had health coverage.
