Unless you want to incorporate as an LLC or S- or C-corporation (and don't do that without legal advice), your self-employment income is included and wrapped up in your one personal tax return, with the business activities on schedule C. A tax return is really just a simple math problem of addition and subtraction, you should be able to follow it if you print it out. (The rules about what to add and subtract can get complicated, but once the numbers are on paper, you should be able to follow it.)
