Since taxes are not being taken out of your checks, you may receive a Form 1099-MISC from the insurance company to use to report your income. If you do not receive a Form 1099-MISC for that income, keep your own records and enter the income as cash payments.
Since taxes are not being taken out, you are being considered an independent contractor. As such, you are considered self-employed in the eyes of the IRS and your income and expenses must be reported on Schedule C. You are running your own business to provide your services.
