You cannot use a Form 4852 to correct a W-2 that you have received. You will need to contact the employer and request them to send you a Corrected W-2. That way the corrected information will be sent to the IRS.
If your Employer will not furnish the Corrected W-2 you will need to call the IRS toll free at 800-829-1040 or make an appointment to visit an IRS Taxpayer Assistance Center (TAC).
The IRS will send your employer a letter requesting that they furnish you a corrected Form W-2 within ten days.
