If your employer had paid it, it would have been included in your W2. Employee cash payments for health benefits.
If they were from your health insurance plan through an employer, they will be included in your W2 and there is no need to enter anything.
I have never heard of life insurance paying these benefits. If this does not answer your question, please reply with more information for further help..
