Do you see the attachment icon as shown in the screenshot? If not, the form is NOT attached.
Form 8453-S will appear in both the forms list in Forms Mode and in the forms to be filed even if it is not actually attached.
If you do see the attachment icon, and the program is indicating there is no attachment, then as @Critter mentioned, you do need to contact Customer support.
