I did my tax form 1040 in a Tax center on a military base. On the section of the form 1040 that says "Paid preparer use only" the person that did my form did not sign. However, under "Firm's Name" he typed Army Fort Hood Tax Center, he also placed the address his PTIN. My question is does he needs to sign or is the information he provided is enough?
