No, the parent does not need to be documented on your tax return. The medical expenses can be deducted for you, your spouse, your dependent or an individual that could have been your dependent except for exceeding the $4050 income limit.

And you are correct - you can include in medical expenses the cost of special hand controls and other special equipment installed in a car for the use of a person with a disability.
