What do you mean? Are you saying that box 1 has two dollar amounts, and box 3 has two distribution codes? If so, I assume that box 3 has a '1' and a '2'? Is there an amount in box 2?

Your HSA custodian may have combined two 1099-SAs into one: one being for a regular distribution (code '1') and the other being for earnings on a previously withdrawn HSA excess contribution (code '2').

If there is an amount in box 2, put it on the 1099-SA that has the distribution code of '2'.

But this is only a guess. I urge you to contact your HSA custodian and ask them why they sent you such an irregular 1099-SA.
