with your personal income tax account. Please include only one
taxpayer’s information and one tax year per fax; and either use form DEX-93 or clearly list the following on your own fax cover sheet:
- Taxpayer Name (name listed first on return),
- Taxpayer SSN,
- Tax Year
- Brief description or any special notes that you want to provide us to assist with the processing of the return.
