If you don't receive the ITIN by April 17th, then reapply to renew the ITIN, but attach the W-7 to the tax return.  If you were responding to the letter telling you that your ITIN was due to expire, you could have sent the ITIN renewal application along with the tax return instead of just the notice.  It's okay that you didn't, but best for you is to not file an extension (since your ITIN is expired), but reapply so that your return is submitted on time.  

But remember, you still have almost 2 weeks.  You may get the ITIN before the time frame expires and be able to file without reapplying.  One other thought:  if you get the ITIN letter on the very last day (or next to last day) of the season, I suggest you mail in your return.  Do not risk an e-file reject to cause your return to be late and possibly lose benefits such as the Child Tax Credit if you are planning to claim that type of benefit on your return.
