My employer contributions are all listed on my W-2, but my insurance company contributes to my HSA account based on daily wellness goals and provided a credit for a fitness tracker to measure these goals. I need to know how to report payment from an insurance company into the HSA. I’ve perused their website and don’t see any tax forms, but do have statements for these incentivized wellness plan “earnings.”
