Yes, the error was mine, as I didn't answer the questions about IRAs correctly. I now see Form 8606 generated when I look through the return. Only Part II applies to my Roth conversion.
My next issue is filing Form 8606 for 2024, when I did another Roth conversion, and the accountant did not include the form. Thankfully, there is no change to my tax liability, as the full amount was included in my income.
