Just need some clarification to your question, what type of visa does your spouse currently have (tourist, student, or working)? If it's a working visa, your spouse should already be eligible to have a Social Security Number (SSN). If she doesn't she won't be able to get an ITIN because Form W-7 is only for individuals who aren't eligible for an SSN.

 

If it isn't a working visa, you enter the details in line 6c. in Form W-7 as follows:

 

  • Enter only US nonimmigrant visa information.
  • Include the USCIS classification (e.i F-1, F-2).
  • Number of the US visa, and the expiration date in month/day/year format even if it's expired. 
  • For example, if you have an F-1/F-2 visa with the number 123456, enter, in the entry space
  • Don't enter the receipt number.
