If you physically live in state A, but your job withheld for state B by mistake, you will need to file a non-resident return for state B, claiming no in-state income, and requesting a full refund of the withholding.  Don't include any extra documents.

 

If the state questions your return and asks for proof of where you lived, exactly what proof that is will depend on your circumstances.  You might be able to use things like a mortgage statement or property tax bill, or an apartment lease, showing where you physically live.  If you used to live in state B but moved out (changed your residency), you might need to show the old lease that you canceled or ended, matching up with dates on a new lease in state A, or the closing documents for selling your house.  You might include the invoice from your movers showing when you moved.  Enrollment documents from your kids schools could also be used, showing you removed them from a school in state B and enrolled them in state A, since public school enrollment usually requires proof of residency.  You can also ask the state that is questioning you, what kind of documents they would like to see.
