I am not sure what you mean exactly by "electing an early termination date to stop residency", in fact, we did attach a "statement for dual status return" which include a declaration that we are no more residents of USA. Also, we included the dates of residency/non residency in the 1040-NR and 1040 forms.
Is there a specific form for that?
